Become part of the PVA-WI community by joining as a Life Member, Associate Member, or Ambassador Program participant.
Life Membership (PVA/PVA-WI)
Available to eligible veterans with a spinal cord injury or disorder, including conditions such as MS and ALS. Life Members have access to PVA/PVA-WI programs, services, benefits, advocacy, adaptive sports, and other resources. See below for more details on PVA Life Membership.
Associate Membership (PVA-WI)
Available to eligible Wisconsin residents who do not qualify for Life Membership but have a significant physical disability that substantially limits mobility and would reasonably benefit from PVA-WI programs and services. Associate Members can participate in eligible PVA-WI programs and activities. Click HERE for more details or to join.
Ambassador Program (PVA-WI)
Designed for friends, family members, supporters, donors, and others who do not meet the requirements for Life or Associate Membership but want to stay connected with PVA-WI. Ambassadors receive opportunities to learn about PVA-WI, receive communications, and participate in appropriate chapter events and activities. Click HERE for more details or to join.

Paralyzed Veterans of America invites you to join more than 16,000 members nationwide and have your voice, energy, and efforts assist others living with Spinal Cord Injury/Disorder (SCI/D). Membership is free and open to qualified individuals.
As a member, you’ll become part of a family of men and women united to make the world better for veterans and all people with disabilities.

Who’s Eligible to become a Life Member?
PVA membership is open to anyone who:
- Is a citizen of the United States or its territories
- Is a veteran of the U.S. Armed Forces and whose discharge was other than dishonorable
- Has a spinal cord injury or disease (SCI/D)
Related Diseases Include:
- Amyotrophic Lateral Sclerosis (ALS)
- Cauda Equina Syndrome
- Multiple Sclerosis (MS) involving the spinal cord
- Poliomyelitis
- Spina Bifida
- Spinal Cord Tumor
- Severe Spinal Stenosis (causing neurological impairment)
- Syringomyelia
- Transverse Myelitis
We welcome veterans with service-connected and non-service-connected disabilities.

How to Apply, Update or Transfer Membership
Simply complete the form at the bottom of this page or download the paper form and mail/e-mail it to us.
We’ll follow up with instructions for submitting your DD-214 and medical evidence of SCI/D.
As medical evidence, you can have your physician complete this Physician’s Statement Form.
If you move, you may be able to transfer your membership to a chapter closer to you.
